Medical facility users are responsible for entering accurate death data and assigning certifiers. The system validates entries against CDC/NCHS standards.

• Enter date, time, and location of death
• Assign a pronouncer using the lookup tool
• Pronouncer must affirm before certification can proceed

🧠 Cause of Death

• Use CDC-compliant language and structure
• System will flag invalid entries (e.g., vague terms like “cardiac arrest”)
• Use the validation tool to preview errors before submission

📎 Documentation

• Upload maternal death forms if applicable
• Upload rare cause documentation for flagged cases
• Certifier must affirm and certify the case to complete workflow
